How to prepare for your treatment

  • Download the Health History and New Patient documents on the right side of the screen and bring them to your appointment.
  • Fertility patients please also download the Fertility Intake.
  • If you have insurance with acupuncture benefits, please bring your insurance card to your first appointment.  (You may submit your insurance information for us to verify prior to the appointment)
  • Wear comfortable clothing that is loose and easy to access above the knee and elbow.
  • It’s important to eat a good meal before your appointment.
